Lisa,
I have a few suggestions when you are working with spoon feedings. Use a shallow baby type spoon rather than a deeper adult type spoon. Similar to when latching a baby to breast, wait till mouth is wide open and then place spoon mid tongue with very gentle downward pressure. Avoid placing food on anterior location of tongue, since with tongue thrusting, it will just come back out. You may also consider gentle but firm pressure under the chin, assisting to keep mouth closed after helping with placing the food on the tongue, to encourage swallow instead of pushing forward. If this is not working well due to gagging, coughing, choking, refusal, fussing... you may need a little help with finding the right texture for food bolus. Sometimes pureed works well, sometimes thicker works.
