Subject: MRSA
in breastmilk
We recently had a mother present with mastitis. Her
two-and a half-week old healthy full term baby was having difficulty with
deep latch 2nd to large nipples. Mastitis had first appeared at the end of
week one . At that time she was instructed to pump and bottlefeed
expressed milk. A prescription was written and not filled by
mom. Mastitis symptoms were resolving. A culture
was done of the breastmilk at the OB/GYN office during the first incident,
it came back positive for MRSA. Mom was notified and returned to
office yesterday for an exam. The mom was started on Clindamycin.
She continues to pump every three hours. Is it safe to have
her resume giving the baby pumped milk? The mom's nares and
skin were cultured yesterday after the 1st breast milk culture came
back. This baby had been receiving breastmilk exclusively all along
and appears healthy. Are there any protocols for MRSA in
breastmilk? Are there different strains in the community than the
hospital? Is there a way to determine the origin of MRSA ? How
