Hi all,

I work as an LC in a children's hospital and the topic has come up about how to properly document medications
the mother may be taking while breastfeeding and/or providing expressed breastmilk for her baby.  We review the medications mother is taking along with the pharmacist, but since it is the baby's medical record, we typically do not
list all the medications the mother is on in the baby's chart.  We document by exception only-meaning if mom is on a medication or substance of concern-then we would mention it, but otherwise, no mention of mom's medications is
in the baby's chart.  We were wondering what other LCs in children's hospitals were doing and what are the legal
ramifications of charting and/or not charting mother's medications in the baby's hospital record.  Thanks.
