This is not yeast. 

Where are the cracks? (On the face of nipple or at the base?) Is there drainage?

Does the baby have a posterior tongue-tie?

Does mom have eczema or psoriasis elsewhere on her body?

I would try a heating pad on low setting over clothes for 5 mins after nursing and as needed between for presumed vasospasm. 

Has she tried pumping for a few days?
