Thank you to all who have responded with suggestions. To answer a few of them:
1) Baby did have several exams to make sure heart was fine and all came back well.
2) Baby is now 8 weeks old, was clipped at 6 weeks.
3) Baby had poor weight gain at his 2 week appointment when a circumcision was done and that is when he came to me. He had jaundice after birth that got better with at home treatment with a bili-blanket and she was not experiencing sore nipples. However when he was seen he did fatigue at the breast and giving up with little transfer. He was able to lift his tongue but not extend; thus the referral I made for clipping after watching his feeds and verifying a click, etc.
4) mom had to wait for clip until heart issues were ruled out, in the mean time she began pumping after feeding at the breast and began pace feeding him after with her pumped milk half an ounce at a time. She began fenugreek as well.
5) got clipped, soreness went down, baby fed better and still now 2 weeks after clip is needing some supplements here and there to stay on track. He comes for re-weighing this Friday as he had regained his weight and began to put more on but he at this time is not exclusively feeding at the breast and staying nourished.

Any input I would love!
