The site to check on Joint Commission's specifications on the perinatal core measure is http://manual.jointcommission.org/releases/TJC2010A/MIF0170.html. The US Breastfeeding Committee has a document available to help hospitals collect the data that this core measure requires at

If your hospital has not chosen this core measure yet, now is the time to ask that they do. There are 11 core measure sets, 4 of which each hospital must choose to work on. The Joint Commission perinatal core measure set will be part of the day long workshop that will be presented at the ILCA conference in July on the mPINC survey. Katherine Shealy from the CDC and I will be offering this workshop to help hospitals improve their lactation care and services.
