Hi All,

 

I am so tired of resistant thrush challenges and have a mom at her wits end.
I am looking for more input on dealing with the tougher cases that just
don’t want to resolve with standard and not so standard treatments.  All
ideas are welcome – please e-mail privately at
[log in to unmask]  At this point mom has done ‘everything’ and
is still having problems.  I have advised that she get a culture and rule
out some type of bacterial thing (staph, etc).  

 

This information comes directly from mom – I have permission to post.  Thank

January 20, 2010

Thrush confirmed, started Nystatin as directed

 

January 23-28, 2010 (6 day course)

Discontinued Nystatin

Started Gentian Violet bid and coconut oil after every feeding

 

February 1, 2010

Restart Nystatin

Started KidzProbio probiotic supplement and Yeast Cleanse supplement

Restarted Gentian Violet bid, started Clotrimazole after every feeding with
occasional Coconut oil 

Per Lactation Consultant advised to start Bactroban antibiotic ointment as
well; the plan is to continue Gentian Violet / Clotrimazole / Coconut oil &
Bactroban x 4 day course, then if thrush persists to see MD for Diflucan 

 

February 2, 2010

Pediatrician stated not thrush, just “microbial overgrowth” on tongue –
stopped thrush treatments and used baby toothbrush on baby’s tongue as
directed which fairly easily removed white plaque

 

February 6, 2010

Nipples red but not sore, started Polysporin after every feeding and
continued for several days, concerned because this was one of the first
symptoms I had in developing mastitis in January

 

February 9, 2010

Well child appointment, Pediatrician verified no signs of thrush

 

February 17, 2010

Slight thrush confirmed by Pediatrician, baby started on Nystatin 2 ml
painted on tongue with remainder to cheeks / swallowed qid  - treated for
total of 9 days (3 days after thrush appeared to be resolved)

Mommy started on prescription APNO nipple cream qid, changed from using
reusable nursing pads to disposable

Family continuing to wash all laundry in hot water with ¼ cup vinegar and
sterilizing all pump parts / bottles / pacifiers / medication syringes in
microwave sterilizing bags

 

March 3, 2010

Baby confirmed with slight thrush, given history started on Diflucan 18 mg
(3 mg/kg) daily for one week

 

March 4, 2010

Mommy started on Diflucan 200 mg tablets, two tablets on first day then once
daily for 13 days

Family continuing to wash all laundry in hot water with ¼ cup vinegar and
sterilizing all pump parts / bottles / pacifiers / medication syringes by
boiling for 20-30 minutes

 

March 5, 2010

Started Gentian Violet bid, prescription nipple cream & coconut oil for one
week; restarted Yeast Cleanse supplement

 

March 6, 2010

Mommy with early symptoms of mastitis (breasts extremely sore / tender,
erythema around right areola and streaking outwards, extreme fatigue,
feeling unwell) – able to treat with bedrest x 2 days, Advil, Emergen-C
supplements, Vitamin D supplements, Probiotics

 

March 8, 2010

Baby completed Diflucan 1 week course

 

March 16, 2010 

Mommy completed Diflucan 2 week course

 

March 18, 2010

Mommy prescribed antibiotics to treat UTI, noticed nipples are slightly
pinker / red after not using nipple cream x 1 week – concerned about
constant use and microbial resistance, also very concerned that thrush will
return, calling OB practitioner to request culture of nipple / areola and
extended Diflucan treatment
