There's a new book in printing, Balancing Breast and Bottle: Reaching Your Breastfeeding Goals, authors Amy Peterson, BS, IBCLC, and Mindy Harmer, MA, CCC-SLP.  It is published by Hale Publishing, and should be available by December 2009, hopefully sooner.  Written in compliance with the WHO Code, it thoroughly covers many aspects of infant separation, including bottle introduction.  It addresses many questions yet unanswered in other texts.  I think it's fabulous--but perhaps I'm biased.  :)
