and very

carefully offer options that might include AIM. However, our recommendations

only go so far and most of us don't have prescriptive rights.

Jaye here: While many of us do take this seriously, sadly, I hear all too
often, as I did today, that moms are recommended fenugreek or domperidone by
the hospital LC or a local LC with NO investigation whatsoever into WHY the
mom has a low supply. Not all LC's or IBCLC's or Dr's take this
seriously...

This is a regular occurrence in my area and honestly it makes me very angry.
It is poor care, improper protocols and unprofessional in my professional
opinion. I regularly deal with moms who have been tossed fenugreek or
domperidone on day 3, 4 or even 14 or more for milk not being in yet - no
history, no investigation, no blood-work. No investigations of any kind.
This is appalling. I have moms having negative reactions to the herbs
because no one took a history to determine the herbs would be safe for her.
I could never in a million years practice this way - I want to know all the
details of her situation - maybe I am being anal-retentive or perhaps
because I am doing my job correctly.

This is a major problem and is a set up for potentially serious health
consequences for mother and baby and situation like this happen with other
pills Dr's prescribe - like the birth control pill. In the past 2 days
alone I had a young mom call me and ask how long it would take the Yas bcp
to drop her supply. I asked her why she was taking Yas as it was my
understanding it had estrogen in it. Her DR gave it to her telling her it
MIGHT drop the supply a bit, but it would take several weeks - not a big
deal. Her supply crashed to almost 1/2 in 2 days and now she is
supplementing with ABM (something she did NOT want to do) and must consider
herbs if her supply doesn't bounce back quickly. And yet another mom was
given a prescription for the mini-pill by the discharging Dr and told to
start it at 3 wks pp to 'get it into her system' - I SAW THE NOTES FROM THE
DR! What are these people doing and why don't they know better? In both
cases I had moms pull out the product inserts and read...they will be filing
formal complaints.

When our Health Care System starts actually taking care of the patient and
really looking into the actual why's of a circumstance THEN we will actually
get somewhere. We have a few really good Docs and IBCLC's out here who do
this - but there are those of us on the outside who are cleaning up the
messes of those who do not bother to take the time to investigate properly,
find the actual root of the problem and then address and correct it
appropriately. No - that is too intensive and makes too much sense. Much
easier to throw a pill at a mom and let the chips fall where they may...
