Evelyn, had a mom with similar problem some time ago.  It turned out to be a 
bacterial infection.  Took a while to heal with a topical antibiotic.  She 
found relief in wearing breast shells between feeds to prevent anything 
touching her nipples.  I wonder if your client is also not experiencing some 
blanching as well?
