Dear Alice:
I understand that sometimes is difficult and feeling with dispair when we don't
get the expected results. The baby boy is 7 days old and by this time we are
expecting to be latching on. It comes to mind some question regarding how
many times he is breastfeeding?, how many wet and soiled diapers?;Does the
infant had any complications at birth? Does the mother is breastfeeding during
the night?
Some mothers experienced delay in breastfeeding that can last hours or
several days for which the mother needs the encouragement and feeling of
confidence that she can breastfeed. She should continue with the previous
advice of utilizing the hospital grade pump, and continue pumping and feeding,
reinforcing that she express her milk at least 8 times in 24 hrs, particularly
once during the night when the prolactin levels are higher. Reinforce the
importance of good nutrition and drinking fluids during day and night.
If the mother is nursing and the baby is not transferring milk she can offered
the milk that has been stored by spoon or small cup.
Establishing let down when the mother has feeling of frustration may be more
difficult to establish milk production for which needs reassurance in positioning
and latching.
