If you have a fax number I can send you a copy of the forms we use.  NOTE: 
they are a 4 page carbon paper.  one for pt, one for records, one for main 
office.  But at least you will have a blueprint:
1. Always make sure you LOOK at the pump number not on the outside case.
2. Most rentals are done weekly, monthly, or every 3 months
3. Use forms that are credit card friendly, only showing the card number on 
the front page, but blacked out on the other pages.
4. Pumps are expensive, get as much info as professionally possible, phone 
address drrivers licsence.  I think we even ask for ss#.
