I have seen this constellation of symptoms with posterior tongue tie. The odd cry, the horseness, cyanosis
--all of it. I wonder if all her babies have had TT. I would also be referring the baby for CST/chiro.

Hi all,
I'm looking for ideas for a mum with an exclusively breastfed 4 month old
bub. Third baby, breastfed previous two bubs for total of 4 months. Very
keen to make breastfeeding work this time.
Bub has gastro oesophageal reflux and is on 10mg (pri)losec am and 10mg pm +
1.3 ml Zantac (ranitidine) pm. Mum is dairy free, soy free, nut free and
has been since 6mths pregnant. Previous oversupply which mum feels is now
controlled. Mum very experienced with reflux as both older children have
it severely, even after middle child has had nissen fundoplication.
At the breast, bub is tipping head back so far that mum can't imagine it's
comfortably let alone by choice. In fact bub fights being straightened.
Bub will also chose this position when not feeding.
Bub is also arching and pulling back but while clamping on breast. Mum's
nipples have been trashed and bleeding on face of nipples. Although nipples
are currently a little better, they are still sore enough that mum is using
breast pads to minimise rubbing from bra, pads being changed at each feed.
Baby sounds hoarse and this has been commented on by mum, by GP and by paed
gastro. Unusual cry / babbling was easily heard over the phone. Mum said
bub currently sounds wheezy but she does not think it sounds like stridor.
Mum comments that bub is often blue around the top lip (older sibling was
the same but doctor said it was 'wind') and skin frequently mottled. The
staff at the hospital recently commented on the mottling (bub in for
bronchiolitis) - mum said it's just how she normally is and it wasn't
followed up on.
Mum is very keen for breastfeeding to stop only when bub self weans. The
degree of pain with the clamping and pulling and the trashed nipples is
making it very hard though. Mum wonders if it's forceful letdown &/or bub
trying to manage reflux coming up while staying on the breast.
Any thoughts most appreciated.
