I think part of the hestitancy about clipping posterior tongue tie is that the 
anatomic findings are very subtle, probably an anatomic variant, and the 
scoring system refers only to the baby.  I think in a profession where we 
preach a mom/baby unit that it does us a disservice to separate the baby's 
anatomy from mother's symptoms and from the impact on feeding.  As each 
person has shared their experience, both baby and mom have been mentioned, 
indicating understanding that the symptoms of the dyad are making the repair 
necessary.  I don't think we can assume that a surgeon looking at a posterior 
tongue tie will have that understanding.  Not every tongue tie needs to be 
fixed.  It may be helpful to develop language to describe the importance of 
the impact of tongue tie on feeding and not just to describe the anatomic 
components.
