Hi Beth,

It sure sounds like a posterior tongue tie to me. I think what everyone is
interpreting as baby's willful "trick" of the tongue to manage a strong
flow, is really the baby losing the latch because of the tongue bunching.
The reason I think this is because you also describe mom being very sore,
wtih bruising/blanching, keeping baby from getting fatigued (I am guessing
some feeds are lengthy but not effective), and loose and leaky seal on
breast. Luckily there have been several recent posts on the clipping of type
3/4 tongue ties.

One way to do a little test, before looking further into clipping, would be
to have mother express off the first ounce perhaps of her milk. Then have
baby attach and see if mom and baby can comfortably bf. If the tongue does
the bunching again, and pushes nipple out, I would go ahead and pursue
further investigation and treatment of tongue tie.
