Does
anyone have a standard letter of this type that I could see or borrow from?
Maybe one with references? I would really like to help this mother out.

Dear Jody, keep it simple. Your M.D. credential is enough for the Judge.
I would just write
Dear Sir,
Please be advised that I am the physician of Jane Doe and her infant
daughter, Carol. Ms. Doe is breastfeeding her daughter and must feed the
baby on demand. I would recommend that the mother be available for feeding
when the father visits the baby and that visits be kept to a maximum of one
hour until the child is about one year old.

I know of a situation like this where the Judge took the physician's advice
and the father and mother met at a public library/community centre complex
for the visits. The mother would go and read in the library and the father
would have a nice visit in the lobby of the community centre where there was
a seating area with couches. He could change the baby, hold it, and
interact but Mom was close by if he needed assistance. It worked out well
in the first year. After that, visits went to 4 hours (child was eating
solids) and increased as appropriate. The mother's lawyer can probably give
you some guidance, too.
