I never registered my son's birth, but I have not run into any problems.  We obtained a Social Security card for him.   If they check the with the US Passport Department there is a list of acceptable alternatives that may be used instead of a birth certificate.  I believe some of them include a baptismal certificate, records of any pediatricians visits, family bible records.  There are several that you can use.
