I agree that more than one method of expression can work well. Many mothers
find a way that works best for them that isn't at all like some of the ones
I've seen or read about or taught. I do remember someone who was the mother
of twins sitting for the IBCLC exam back in 1985, who was in the restroom
hand expressing on a break to relieve the pressure, telling us that she got lots
more milk out faster by hand expression and this was before the Marmet
technique as widely known.
At the Birthing Center where I work I teach the Marmet method (that looks
like the one shown in the Russian site's video) with the pads of the
thumb/finger pushing in and then rolling down onto the areola, but find that some
women's thumbs don't bend quite the same angle and they find it awkward. So for
them I use a method that was in the 2nd ed. of Riordan, I think which showed
placing the thumb and opposing finger on the breast near the edge of the
areola, then spreading the placed thumb and finger further apart which essentially
cause them to press slightly inward, then to compress the breast. It works
well, too.
I have a dear friend ( with a number of children -- all breastfed) who
didn't get much at all using the Marmet technique with the fingers close to the
areola. We were having a discussion about pumps as she'd rented one to help
provide milk for a friend's sick baby. When she showed me her manual
expression method, milk sprayed out abundantly and quickly. She took hold of her
breast with thumbs on top -- further up on her breast than the edge of the
areola, and the all the rest of her fingers opposite on the bottom of her breast
and then compressed the breast rhythmically and milk streamed out so fast -
much faster than either the Marmet method or the hospital grade pump she also
demonstrated for me. She was using the pump so she could collect it easily --
and read while getting the milk.;-)
