And don't forget the possibility of Paget's disease if the nipple pain 
and increased size fails to resolve with normal measures. This does 
sound like a bleb, perhaps with an extensive plug behind it; but if the 
white area looks more crusty or broken than smooth and blistered over, 
she should consider a biopsy for Paget's.
