Thanks for your responses.  I must have been very tired yesterday not to
have remembered "the HIPPA paper"!  :-)  Of course patients sign it when
they are admitted to the hospital and we have them sign it again if they
come back to our office for a visit.  Laurie, we do things pretty much as
you describe, except we already have our consent form for sharing info with
the physician in place.  One nice perk that we also have is that we have
been able to arrange with our county WIC office for a peer counselor to come
and do rounds a few days a week at our hospital.  She sees any WIC patients
that we can identify and it's great for them to have that extra support
right from the beginning.  I think that makes it even more "legitimate" when
we communicate info to their office (by HIPPA standards).
