become failure to thrive or die?"

Professionals have a moral obligation to identify deficiencies in their
knowledge or skill base.  This facilitates two important processes.  The
first is referral.  The second targeted continuing education.  
When health professionals fail to identify (and rectify or refer) the gaps
in their knowledge/skill set, they are neither doing their best nor are they
providing care that is good enough - and they should be censured by their
professional organisations.
