Dear Listmates,
I am very interested in learning if the hospitals you work in currently use
written consent forms for nipple shield use and/or pacifier use. If you can
e-mail the forms as a "cut and paste" I would love to see them.
I have seen a written consent form for giving formula when a mom requests it
and there is no medical need. This was from Melrose-Wakefield Hospital in
Melrose, MA.
