Is there anything unusual about the mom's nipples?  If for some  
reason the areolar tissue is very tight and inelastic, this could  
cause the baby to not be able to draw the nipple far enough back, but  
I would think this would gradually improve, unless there is scar  
tissue from surgery or something.
