This poor mom. Sounds like the pregnancy might be in jeopardy, and the
doctor has set her up to consider it her fault for breastfeeding her
toddler.
Breastfeeding through pregnancy has not been shown to increase the risk
or miscarriage. If the pregnancy is healthy, all should be well. If it's
tenuous, then weaning probably won't protect it.
When I talk to moms about reproductive matters and breastfeeding
(partially or fully weaning to get pregnant again, bf through pregnancy)
I usually gently open a discussion about the baby she has vs the baby
that might or might not be. I might say something like "it can be hard
when you want to do the best for the baby you already have, but you want
another one too and you feel your biological clock is ticking". This way
mom can sometimes come to clarity about what's her priority at this
moment. It might be different from my priority, but that's her right.
For this particular mom, you might talk about how miscarriages are
common, that they are no one's fault, that they are unavoidable if the
fetus is not healthy, and that nothing one does can stop one or cause one.
