Here is a position that may work for this mom. I have used it in my area  
successfully. Put the baby more at an angle. The baby's body is sitting up more  
(like a cradle hold but on an angle). A pillow under mom's arm (that is 
holding  the baby's head) will help keep mom comfortable or use the side of the 
couch.  Lean mom back quite a bit on a comfortable couch or lounge type chair if  
possible. Put her feet on a stool. If the baby is in a cradle hold the mom can 
 pick up the underside of her breast and raise it higher into the baby's 
mouth.  Remind mom to keep her fingers far back on the breast and not near the 
baby's  mouth. Adjust baby for comfort. If the baby is gagging when breastfeeding 
mom  can compress in different areas of breast to stop or start milk flowing. 
I find  that the angle makes some baby's happier to stay on the breast vs 
being on a  laying down position across the stomach. I hope this helps.
