It can often be challenging to eliminate all sources of gluten. A dietician
who is knowledgeable about the celiac diet may be helpful in tracking down
any hidden sources of gluten- sometimes things such as vitamin supplements
or medications that are not gluten-free could be causing a problem or there
could be a cross-contamination problem. A useful reference is "Gluten-Free
Diet: A Comprehensive Resource Guide" by Shelly Case, R.D. (revised edition
published 2006). Has this patient been on the celiac diet for very long?
Sometimes it can take awhile for the GI system to heal. Or, there could be
something other than celiac contributing to this woman's symptoms?
I don't recall having seen any information about lactation worsening
symptoms in a treated celiac patient. I have heard anecdotal reports from a
couple of mothers who said that their untreated celiac disease worsened upon
weaning and that is when they were eventually diagnosed. They speculated
that the hormonal changes associated with lactation may have helped reduce
the severity of their symptoms.
An Italian research group, C. Ciacci et al., studied celiac patients during
pregnancy and postpartum. The study is published in the Am J Gastroenterol
(1996) vol 91(4): 718-22. The authors report that breastfeeding duration
was lower in patients who were not on a GF-diet, but not in treated celiac
patients.
