Hi all
Thanks for the encouragement so many of you sent me.  I have written the
letter and posted it on bmj rapid response.  (I am in very good company,
sitting next to Marsha Walker and Rachel Myr!!)  It looks pretty aweful -
the web page takes out all of the carriage returns and makes it into one
huge block of text (including references).  Hopefully the online reviewers
will notice that it is a properly referenced paper and publish it in the
print edition as well.
The title of the response I sent is, "No cognitive advantage for
artificially fed infants given some breastmilk".  Let's see if it flies.
