I have a young mother whose child is 7 months old. She returned to work
when the baby was 5 months old, and breastfeeding dropped down to 2-4x per
day, for about 5 minutes each session. At the same time, mom resumed birth
control pills and started the baby on baby food. At age 5 1/2 months, she
came to my office for a consult mom was concerned that her milk production
was low. After encouraging her to try to lengthen her nursing sessions, I
encouraged her to increase her pumping to every 2-3 hours while awake and
at least every six at night. Mom was already pumping throughout the day so
that baby could have EBM while she was at work. Mom has an excellent double
electric pump and verbalized correct use. I verbalized to mom my hope that
if her milk supply increased the baby might not be as interested in
the "other" baby food. Mom said she'd call if she ran into any trouble.
Now, 1 1/2 months later, mom calls to say that her milk is completely dried
up and she wants to breastfeed again. She reports that the supplementer
nurser never worked and that, despite pumping every 3 hours for two weeks,
she cannot obtain even a drop of milk. Mom is trying hard to do the best
for her little girl, and I don't want to fail her. But she states that now
the baby won't breastfeed, the supplementer won't work, and the pumping
isn't working to bring back her milk supply. What else can I do for this
couplet? Is it worth referring mom back to her OB for prescription meds at
this point? Would herbals be of any help in this situation? Any suggestions
