Hello!! I joined the list a few months ago and have been so impressed by
the wonderful wisdom of you all! I'm inpatient now on a mother-baby floor
after spending years in the community and then 6 years at home full
time.I'm in the process of re-educating myself-- I would love to tap into
your collective wisdom on the following issue--
Mom diet controlled gestational diabetic- 3rd baby- breastfed other babies
without any problems. C/S after hours of pushing- baby 9 lbs 2 oz, 39
weeks.Mom with temp in delivery which means baby got an IV and a workup-
started on Amp and Gent. It's now 1 hour post delivery- mom in recovery
room. Baby's 1st glucose by strip is 34. Parent's absolutely want no
formula- despite nursery nurse's hesitation- Dad's great advocacy got baby
out to mom for a feed. 10 minutes on breast with good latch according to
labor nurse. I'm still in orientation so I'm observing and taking all this
in.
Back to nursery for IV meds and repeat glucose- 40 mins after feed-
glucose still 34. Neonatologist called- baby admitted to NICU for IV
glucose. Mom of course unable to see baby- she's a fresh post-op. NICU
doesn't bring baby down to feed- mom must pump- not sure what method they
will use to feed baby.
So- I overheard the MD telling Dad that colostrum doesn't have what the
baby needs in terms of getting the glucose up- "there is not the volume
needed to counteract the dropping glucose due to mom's gestational
diabetes."
Please advise me in how to best advocate for these mom's and babies and
what is the physiology of the situation- and the content of colostrum for
bringing glucose up....... I haven't had a chance yet to do a lit search
but plan on it.
