Hi Kathy,
A couple years ago I attended a conference with Rebecca Glover as a speaker.
She talked about management of engorgement with the use of expressing. Her
theory and observation was that if the breast is not relieved of the
engorgement either by the baby or expressing, the mother could loss some of her
ability to produce enough milk. Than there is just the comfort factor.
She described that when the breast were engorged and the baby would not or
could not reduce the breast, the mother was to express them down to being
soft. She stated this would only be needed for a few times a day over a day or
two. After that the engorgement would be resolved and the mother comfortably
breastfeeding. She stated this management would not lead to over production.
I have used her advise on a few cases and it does help. Some moms like the
idea of building up a supply but I discourage them to continue this and just
resume breastfeeding their babies. None of these moms have ended up with
over supply issues.
I don't know if Rebecca is on LACTNET but maybe someone who knows her can
comment on this.
