Hello Lactnetters:)

I am working with a mom who has given me permission to post.  Baby girl was
born 3-15-06, birth weight 8 # 3.0 oz and length 19 3/4.  She had difficulty
with latch in the hospital and she was given a nipple shield by staff.  Mom
recalls the shield being given "right away". Her discharge weight from
hospital was 7# 10.0 oz.  Her present weight is 12# 12 oz. and she is 25
inches long.  Mom contacted me one month ago really wanting to wean off the
shield, she is just plain tired of using it and I am concerned about milk
supply.  When I went to visit mom one month ago she was only nursing babe
about 6 times a day.  We tried a few things that day with the shield, first
starting the feeding with it on, then trying to slip it away, but this baby
was so curious while I was there that she just wanted to look at me and not
really nurse.  Then we tried having mom pump before latching to see if the
pump bringing out the nipples or aiding in letdown would help.  My
suggestions to mom after that visit were to increase feedings to 8 times a
day. I also suggested to try at times when baby was sleepy or just waking
from a nap and to take baths with her and do a lot of skin to skin on her
chest, I also explained to mom to not make it a time of frustration, so when
baby starts to get upset to stop attempting without the shield.  Also
watching her pump I felt the flanges were too small, so she is now using the
Large flanges with better success.  She also did some pumping for 10 minutes
after nursings to increase supply and is taking the Mother's milk tea.  She
feels baby is happier now and nursing up to 7 times a day, but still cannot
get her off the shield.  She even tried putting a piece of cloth in the
shield as suggested in the Breastfeeding Answer book, but mom said she got
so mad so she stopped that attempt.  The last thing I suggested to mom was
to give it time,  and when trying to keep it low key and if she begins to
get upset quit right away.  Mom has also visited kellymom.com and has done
suggestions on that site.  Baby's doctor is not worried about her weight,
she is 5 months old now but developmentally doc felt she is at a 6-7 month
level.  Baby's father is very tall and lean and from Africa, mom is
caucasian, normal height and build.  Baby is very bright eyed and curious
and looks great.  Any suggestions? Is it near impossible to wean a baby off
the shield this late?  I'm just wondering if there is another weaning
technique that she hasn't tried.  Thanks for any help.
