I have mother's permission to post this info. This mother's first
baby was born with Treacher Collins Syndrome but an unusual form. He
is now three and one of his problems is a sensory integration
disorder. He has problems eating - as his mom puts it - he has "no
drive to eat" I helped her initially to get this little guy
breastfeeding and he finally latched well well at 3-4 mths of age. He
nursed for 13 months except for a period of time when she was pumping
and dumping due to what she described as massive doses of Diflucan
for "presumed" yeast infection in nipples and breast. I say presumed
because she was not initially treated very aggressively and it
sounded as if her thoughts about a yeast infection were not taken
seriously. I was not with her at this time - she had moved away.
After this period of Diflucan she had about 4 mths of being pain free
then it started again and she toughed it out until she weaned her
baby. She herself had her first yeast infection at the age of 17 when
she was catheterized during a surgery (can't remember what for)
following which she developed a bacterial infection then a yeast
infection. Her first baby was delivered by c/s and the same thing
happened with that catheterization. She seems to be a walking yeast
infection from her description of herself with infections in her
mouth and other mucous membranes. Another thing she mentioned was
that about half an hour after nursing, her nipples would be white -
not as I would visualize them in Raynaulds but she said it looked
like a dusting of powder. Her baby has never shown signs of a yeast
infection in his mouth, but, and this is my presumption, perhaps as a
diaper rash. She is pregnant again and determined to breastfeed but
so worried this might all happen again.
She is scheduled for a R. c/s in April. She is leaking colostrum and
like last time her nipples are covered with a green crusty substance.
She says her nipples are very tender and she does have an occasional
sharp pain in her breast. Upon further questioning she says this
green crusting is not really a scab it is more as if the nipple
tissue looks like green cauliflower, and is about the size of her
pinkie fingernail. She did have this problem while breastfeeding
before. Her obstetrician has not helped her, hence her getting back
with me. She is due to deliver at a fairly small hospital without an
LC. I initially suggested she might find a dermatologist who might a)
be sensitive to her very strong desire to breastfeed and b) might be
able to come up with a diagnosis. I thought that it might be a good
idea to try to get a culture and sensitivity done, but then it
occurred to me that I have learned so much from Lactnet that perhaps
some of you have come across something like this. I must confess that
all this info was from a 'phone call this morning from 100 miles away
and no, I have not seen her personally.
I am really looking forward to hearing what you think.
Thanks in advance Susan Brewer RNC IBCLC
