Does anyone have an evaluation/assessment form (checklist format?) 
that can get put in a patients chart that they can share?
I'm just starting a hospital LC position and I feel this is one thing  
that might help.  Currently they have a blank
piece of paper to chart your findings but I feel it might be faster/ 
easier and more helpful to have a checklist format followed by a  
blank space for comments.  I know I've seen one like this somewhere  
but can't remember where.
