I think I stopped reading after the first 2 paragraphs to send this, and especially when I read "peau d'orange" - get mother to a knowledgeable breast surgeon or breast cancer specialist asap - now. She needs a biopsy.
This has a high probability of being IBC.
Other notes:
With multiple repeated courses of antibiotics with little or no change, one must consider that maybe it's not infection. Refer to breast specialist.
Any mother whose signs and symptoms of mastitis do not improve in about 24 hrs, or who is getting sicker or signs and symptoms are increasing or severe, should be SEEN IN PERSON by a primary provider. Also consider, even is a mother has/had MRSA, with many courses of antibiotics, this may not culture out.
Please, LCs, do not continue to try to manage these moms, unless you are also a primary provider. Of course, continue to support her lactation, when possible, in cooperation with the primary provider.
I didn't follow the baby's course too closely, but IMO, going from 6#0 to 6#8 in 18 days is a not a good weight gain.
I hope at this time, mother has found the right healthcare provider and begins to get well.
