Without seeing this baby, I would not really know how to advise you and 
mom. But, I have found that it takes 3-4 treatments when baby is being 
seen by a craniosacral therapist. Not just once. If baby is a jaw 
clencher, mom will have pinched nipples when baby comes off the nipple. 
Make sure baby is not doing a "fish lip" latch-on or the tight mouth 
that looks like you blowing a kiss. Corner of mouth should be wide, like 
a U not a V. Pumping should not hurt, but if she is on too small 
flanges, it can. Mom might need bigger flanges, or a better pump 
(meaning rental quality). She should turn the suction down if she has it 
on max. A good latch on technique is to pull baby close to mom's breast 
and put the nipple on his top lip. Wait until he opens wide and pull 
baby in quickly but gently. If her hand is behind his neck, she can 
achieve this easily. If baby's head rests on mom's arm, it will flop 
around. Also, baby needs to have test weights done before and after 
nursing with the proper baby scale to assess what he is taking in, both 
with and without the nipple shield. Amounts of sucks before swallows is 
not accurate enough to assess intake when baby is not gaining weight
