We had a local, well respected paediatrician who regularly had mothers wean
their babies to formula for "lactose intolerance" when he saw their normal
breastfed stools, which tested positive with a dipstix for reducing
substances. He's retired now.
And when I breastfed my baby, I was worried by her stool, reassured only
because I had a copy of "The Womanly Art of Breastfeeding" which described
them as even looking like pea soup!
It's lack of training and experience.
