I am posting today, with an urgent need, of evidence based research
studies.

Our hospital based Lactation Office, with 3 IBCLC's, provides care at a
facility with 1500+ births per year and has a 24 bed NICU. We do both
inpatient and outpatient consults.

We have a baby currently in NICU, that was given a diagnosis of
suffocation on mother's chest by breast tissue. This was given after other
possibilites ruled out. Prognosis of the baby remains uncertain.

This was an elective induction at 38 weeks gestation. No pregnancy
complications. Negative group B-strep status. Mom was G2P2 and of average
size build, who nursed previous child for 10.5 months. Epidural was placed
for labor pain control.

Male child delivered without complication. Apgars 9-9. Baby not separated
from mom after delivery. Put to breast at some point in cradle hold
position. Mom draped baby blanket over baby's head for privacy, with
visitors present. At 1.5 hours age, baby found lifeless without pulse or
respirations, when her nurse pulled back blanket for observation. Mom
unaware infant was in distress. No infant struggling noted per mom. Full
code began which restored cardiac activity and vent. support required for
respirations. Baby now off vent with posturing/seizure activity and
abnormal EEG activity. No intracranial bleeding.

We are concerned with this diagnosis and the implications of such. Is this
even possible in a baby with supposed normal neurological status?
Shouldn't a normal infant struggle with suffocation? What factor could the
epidural or the blanket have played in this tragic event?

This case is under current review and we are desperately in need of
research studies or references. Please post any assistance publicly over
Lacnet, so my associates can view also.

Thank you,
Lynn Foster R.N.,IBCLC
