I would bet she is having vasospasm. Her nipples were chewed on non-stopfor
at least the first month, and off and on since then, and it has been my
experience that babies' latches do not sudeenly improve at 1-2 months, but
their little mouths grow large enough that a poor latch is no longer painful
to mom. But they can still be causing tissue damage.
The good news is the symptoms of vasospasm are relatively easy to treat. A
heating pad on low applied to the covered breast immediately after removing
baby for ~15-20 minutes to prevent the vasospasm.
The bad news is it is next to impossible to get a baby to latch more deeply
at this age. If mom has an overabundant milk supply, baby may be encouraged
to latch shallowly to protect itself. I have had some luck with applying a
shield for a while to protect baby from hard spray while reducing
supply...but it does not always work. (older babies in this situation tend
to refuse to latch to shield at all, or just get onto the tip)
I do not think it is yeast. I rarely see yeast stop at the nipple/areaolar
junction. Since she has eczema elsewhere, you could suggest trying 1%
hydrocortisone ointment (not cream) to nippoles after every feeding/pumping.
If she does have multiple nipple blebs, that would support the theory of
long-term mechanical damage. Pictures would be great.
