These are my suggestions:

culture mother and child's nares for Staph aureus  and treat monthly with 
bactroban if either positive

longterm antibiotics:  for the next four weeks at full dosages followed by 
low dose therapy with erythromycin or bactrim


Consider slowly weaning from the affected side while continuing to breastfeed 
from the other.
