It may be a long trip but there is someone here near NYC who evaluates  
frenulums in need of release, even those 3 and 4s no one else will look at or  
recognize. The release is a quick deal with a little topical for numbing, a  brief 
holding with gauze and  pressure and then right back to mom and the  breast. 
Some babies latch for the first time ever, within minutes, other latch  better 
than ever, some it takes more time, but always feeding overall is  improved. 
Weight gain being good is terrific, but as we all know it's not the  whole 
story, and sometimes peters out as time goes on, and baby  can't "surf" anymore 
on a large supply/flow.
Also restrictions which  don't make a difference to weight gain can still 
make a comfort difference to  mom, an energy difference for the baby, as you 
noted, and future differences in  speech and oral hygeine ( can't get those poppy 
seeds out...:) I know a family  came all the way down from Canada not long ago 
to see this pediatric surgeon.  It's a long way from Indiana, for sure, but 
if this mom has any relatives in NYC  or Long Island, now might be a good time 
for a visit...
