I have know several women who can breastfeed really well, baby is
gaining well, but they have trouble pumping. I believe this is because
the breast is smarter than we think and doesn't recognize just suction
to let down. It wants the baby! I would have mom practice, practice,
practice. She may need a rental pump, or a Purely Yours (or Lansinoh
style) or Pump in Style. She may need larger flanges. I would have her
breastfeed at home and then pump afterward for 5-10 minutes double. Or
when nursing the second side, pump the first (nurse the first side, then
go to the second, and pump the first after nursing on it.) This may help
get her body used to the pump. I also find that many women do better
with a good manual pump than on the electric! I would suggest a Medela
Harmony or Ameda one hand because you can get larger flanges. The Avent
ISIS is nice but the insert makes the tunnel small for many women.
Single sided pumping is five minutes on each breast going back and forth
to do each breast 3-4 times, or 30-40 minutes total. Oh, and I find some
women do not respond as well to the computerized pumps, like the
Symphony or the PIS Advanced, as they do to the old fashioned styles
that you change by hand. If she is using one of these two pumps, she
should try a Lactina or Elite or PY or PIS regular. I would strongly
suggest a rental over a purchased pump for any woman having trouble with
a purchased pump, even the more expensive ones. And if she is using one
of the new "massaging" pumps (Avent or Playtex), she may not feel it is
comfortable enough to let down to. Some women do not like this massaging
action, but others do.

PS: If she has a rental or a used pump, someone should check the suction
with a vacuum gauge and make sure it is working properly.
