Funnily enough, I am in the middle of a writing assignment for the
day job, re-writing a breastpump/breastfeeding accessories
manufacturer's website copy. Even without the re-writing, the
manufacturer recommends nipple shields to be used under professional
guidance only. Maybe this would help you write a protocol - if a
mother is using a shield, the professional should be around to
oversee it, and to oversee the baby coming off it, too. This is the
guidance in Woolridge's Enabling Women to Breastfeed, if you want a
reference.

That sounds very risky to me - it's not what the things are designed
to do, and a baby might conceivably draw out enough cotton threads to
choke.
