This sounds like more of a baby problem than a maternal problem. If mom 
is able to pump significantly more milk than the baby just took, that 
means that the sucking is very ineffective.
Probably the milk supply declined as the baby entered energy 
conservation mode from underfeeding. This tends to spiral downward - too 
few calories = sleepy baby = poorer energy for feeding = fewer calories 
= more energy conservation and less active suck = fewer calories, etc.

Lots of pumping should increase it, and I'd definately try supplementing 
at breast.
