the culprit if this cleared 
up the minute the babies went from breastmilk to formula.  ...My
assumption has been that baby is getting a whole 
lot of lower fat milk, making for more irritating stools...

This is an excellent point. The problem is that with this retrospective
report, mom's memory may have been influenced by her interpretation of
events. In the one baby in particular, it sounds as though we can assume
the problem started at birth, since after trying "hundreds of dollars of
creams" the baby is finally weaned at three weeks. The other baby didn't
go much longer. We have to wonder if the rash cleared up coincidentally
with weaning rather than because of weaning, and if something is
occurring around the births that is precipitating the rash. Exposure to
antibiotics, exposure to a hospital environment...perhaps that is the
length of time each baby was in paper diapers before going to cloth, or
before moving from a smaller newborn brand of disposables to a larger
size in another brand.

Becky has an excellent point, though, and foremilk:hindmilk imbalance
and/or oversupply should definitely be looked into. However, there is
also the possibility of an opportunistic yeast infection, and I would
still consider using an antifungal now while the investigation proceeds.
Personally, I would also switch to using double-rinsed cloth diapers,
using running warm tap water to rinse off the baby's bottom at every
[frequent] diaper change rather than wipes, and discontinuing the heavy
creams during yeast treatment.
