Another question to the Wise Holders of Breastfeeding Knowledge...
My client has no pain. Breastfeeding is comfortable and baby is
transferring over 3 ounces within 10 minutes. (Oversupply may be an issue, but last
baby had weight gain issues and low milk supply. Don't think mom wants to
tamper with milk supply.) During the feeding, the baby repeatedly 'pops' off.
Pop is the sound you hear too. Smacks the tongue....which has no frenulum
issues and is within normal range for length. Poor baby is extremely fussy after
all the feedings because of this constant air gulping due to not staying
sealed. Mom says she can 'hear' the bubbles while the baby gulps.
Mom did try expressing to see if bottle feeding would be easier for baby,
but no. Popped off and on bottle, made some 'air swallowing' (mom's
description) sound and still very uncomfortable after feeding.
Mom is feeling overwhelmed with this unable to settle baby who is gaining
like a champ. When I first saw her a couple of weeks ago, I noted the baby did
seem to be trying to control for flow and her mer is not overwhelming, but
is strong. She is very large breasted so feeding on her back won't be very
comfortable for her or the infant.
Other than the typical tongue exercises mentioned all the time on Lactnet
and in the BAB, is there any other things that may encourage the baby to stay
sealed at the breast? I did mention she may want to ask her pediatrician for
a referral to a specialist, but in our area, well, nos so sure we even know
one who is 'breastfeeding knowledgeable'.. I guess she could work on the
suck and the seal getting right and I could worry about transferring that
behavior to the breast if a specialist could only work with the bottles.
Looking for ideas...
Thanks,
Pam MazzellaDiBosco, IBCLC, RLC
PS On an unrelated topic: Any idea why I am seeing so many higher palates
with an odd little bubble area right behind the ridge? Nipples of late have
been enjoying themselves in that spot and poor mommies have very damaged
nipples. Thanks to the higher palate and the little spot we have to work very
hard to get even more breast in. Why did I see 5 of those this month?
