I wonder if the baby has been checked thoroughly for hernia. It's a shot 
in the dark, I know, but I have seen two babies who were diagnosed very 
late with hernia. My niece was into her 3rd month of age before she was 
finally diagnosed and admitted for surgery and this latest little boy, 
who's mother I was helping with her supply, was 5 weeks old.  Could be a 
chicken or the egg thing but if her baby started out fussy and screaming 
because of low milk or inability to handle the formula she may have caused 
a hernia which is now the culprit.  Or it may have been there all along. 
Another little boy would scream so much at 5 days that I suggested the mom 
immediately see her pediatrician and not wait for the two week check up 
and he also had a hernia. Neutramagen helped him also, the Similac made 
him scream, too.  She could only produce 3 ounces a day, though so after 
one month switched to hypo-allergenic formula only (milk insufficiency, 
pendulous breasts, bulbous aerola.)
