I've seen this when baby has a tongue thrust and the latch is shallower 
than it looks. The baby's tongue tip presses on the junction of the 
nipple and areola, and breaks the skin there.
A more extremely asymmetrical latch and perhaps some work to reduce 
tongue thrusting (refer on to an occupational or speech therapist if you 
are not experienced with this) should help.
