This is a question that frequently comes up with adoptive moms, who have 
either some amount of frozen milk that they have been able to pump in 
advance of their babies' placement, or some amount of donated breast milk, 
but still need some amount of formula supplement.  What I have done, 
personally, is prepare two Lact-Aids, one with an ounce or two of EBM and 
another with a little formula.  If I think the baby still needs more 
supplement, after the EBM is gone, I can quickly get the second Lact-Aid in 
position, before unlatching the baby from the first one, so that it doesn't 
require disturbing the baby.  If the baby seems satisfied with the amount of 
EBM, with whatever he is getting from the breast, I just put the one with 
the formula back in the fridge.   I have also done the same with bottles, 
before I got Julia nursing.  Having to wash an extra bottle or Lact-Aid is 
preferable to having to disrupt the baby in the middle of a feed, or putting 
both EBM and formula together and risking not having the baby take it all.

Another thing I did at times when I was only able to offer the baby a few 
ounces of breast milk a day, was rinse out any container that had EBM in it 
and save the water for mixing formula.  Of course, I only did this with 
containers that we had just barely finished with, and not any that had been 
sitting around.  Probably not something one would ever do in a clinical 
setting, though!
