Just one more thing to add:  I worked with someone a few years ago and all
of the suggestions made here were also made for her:  Using a sling;
adapting it; sidelying; special pillow; etc.  But one thing no one has
mentioned that helped her was sitting on the floor to nurse.  My client was
worried about dropping the baby and she had much more confidence sitting on
the floor.
