I agree with Cathy, 2-3 ounces 10-12 times per day should be enough,
especially if mom allows the initial let-down to subside (into a bottle or
towel) and then puts the baby on.  The slower flow after the let-down
subsides should help with this baby's choking problem and will satisfy him
more as well as put the weight on.  If mom has an oversupply (I couldn't
really tell if this is the case from the post, but hyperprolactinemia was
mentioned and the baby got a fair amount in less than 5 minutes) and the
baby is self-limiting due to his breathing/choking problems, he may not be
getting enough of the higher fat milk.  There was no mention of whether this
problem has been ongoing (baby is 12 weeks) so I wonder if this has been
happening since birth or is a recent development?  That may change things
depending on which it is.  The baby was in the 75th percentile but now has
dropped to the 50th, but has not gotten formula since discharge from the
hospital, so how was he feeding before to be in that higher percentile?
What has changed, if anything?  I can't imagine it would be a tongue-tie
issue if these problems are fairly recent in development.  Also, lots of
preterms/near-term babies have reflux issues (I know, I know...there goes
Marsha and reflux again <<eyes rolling>>) and that is what the
cereal-enhanced formula was tried to decrease, that or swallowing issues.
But again, at 3 months, it's a little late for these symptoms to just be
showing up.  Any more details you can share?
